Traffic lights out of action and no police?

Three times in the last couple of weeks, I have noticed traffic lights out of action at a major junction in the area and no attendance by the police - twice at Gipsy Corner and once at Ealing Common. It is very scary having to take your life in your hands and try to cross the A40 with no pedestrian crossing, not to mention the huge traffic build up on either side  - especially from Victoria Road. It's a pity that in these circumstances the traffic on the main road (eg A40) can't be more sensitive to the needs of pedestrians who have been waiting for 20 minutes or so to cross. At Ealing Common, I was a driver myself and again it was scary to edge out across oncoming traffic moving at reasonable speed. Surely some agency is respoNsible for these crossings and responsible for getting the police to step in when the lights are out of action?

Was the A40 Wales Farm Road crossing closed, with all traffic directed towards East Acton as in the past? As for crossing the A40 on foot, I would have considered walking either to the footbridge or the other side of Horn Lane / Victoria Road.

The entire crossing at Gypsy Corner had no traffic lights.  There was no warning about this at North Acton station or anywhere else, and you did indeed have to take your life in your hands and cross the 6 lane A40, in the dark, during the rush hour.  On the first night at about 6.30pm I spotted a police van and reported it to them.  They said they had been told about this at 4.30pm but obviously nothing had been done.  On the second occasion I dialled 999.  It was so dangerous, and I am surprised nobody was run over.  Would it not be too difficult to get traffic police up there to manage pedestrians crossing and direct them to a safer alternative, or to ask London Underground to announce it on the tube.

Was it Gypsy Corner (Horn Lane to Victoria Road and Park Royal Road)? The TFL website said it was the Wales Farm Road to Leamington Park junction, top of the hill.
